  • 6/2/09 - EFS Application Processing Changes (PDF)
     

  • 5/15/09 - A new EFS form  has been created that incorporates 2009 RCG data and line item revisions into Appendix B. Line items may be copied from Appendix B directly into the EFS form. Guidance to complete the EFS, previously available on the OPS website, has been gathered at the bottom of Appendix B. A new data validation feature warns the user if incorrect values are entered for rates or markup. Formatting has been streamlined to eliminate one column and allow quicker navigating within the pages. Comments or questions may be directed to Tom Fox at 303-318-8535.
     

  • 4/29/09 - This report format, with all applicable attachments, may be used to satisfy the requirement for submittal of a MTBE Assessment Report.  Each field of the three tabs must be completed, and all required figures, tables, and other supporting documents must be included with the submittal.  Click “OK” on the circular reference box (Excel is searching cells for data that hasn’t been entered yet).  Mail two copies of the report to State of Colorado, Department of Labor and Employment, Division of Oil and Public Safety, 633 17th Street, Suite 500, Denver, Colorado 80202-3660
     

  • 1/1/09 - The new Site Summary Form (SSF) is now available on the OPS website.  The new SSF must be used for all petroleum releases confirmed on or after January 1, 2009.  The former version of the SSF will remain on the website for releases confirmed prior to January 1, 2009.  Tank owner and operators with releases prior to January 1, 2009 have the option of using the new SSF form which has a due date of 60 days from the date of the release and allows for a risk-based SCR due date. 
     

  • 3/6/08 – The Division of Oil and Public Safety, Remediation Section (OPS) has updated the Monitoring and Remediation Report (MRR) format.  Following an extensive review and testing by the OPS staff, the Colorado Department of Labor and Employment technology department, and OPS consultants, additional features have been added to enhance the efficiency and security of the document.  All monitoring reports must now be submitted in this format.  Please review the instructions prior to entering data into the workbook.

  • 12/11/07 – The new Petroleum Hydrocarbon Vapor Intrusion Guidance Document is now posted in the Documents Section of this website.  Contractors should review this document prior to installing or sampling a soil vapor or sub-slab vapor well and prior to sampling indoor air.
     
  • 9/25/07 – Contractors must include a meter to record operation time on any mechanical remediation system installed at Division of Oil and Public Safety (OPS) regulated sites.  Operation time is requested on the new Monitoring and Remediation Report (MRR) format.
