ATTENTION!

Beginning  August 15, 2003 all Reimbursement Applications received by OPS will be returned without review under the following conditions.

1.  The reimbursed amount to date plus the current application(s), exceed the sum of all costs listed below:

• Economic Feasibility Summary (EFS) costs included with the approved Corrective Action Plan (CAP).
• EFS costs included with all CAP Modifications.
• $50,000 allowed for costs related to site assessment and emergency response.

2.  There are no costs listed in the approved Corrective Action Plan (CAP) and the reimbursed amount to date plus the current application(s), exceed the sum of all costs listed below:

• EFS costs included with all CAP Modifications.
• $50,000 allowed for costs related to site assessment and emergency response.

• Economic Feasibility Summary (EFS) Form Frequently Asked Questions.
   
• The new Economic Feasibility Summary form is now available!  This EFS is required to be submitted with all Corrective Action Plans (CAPs) and CAP Modifications beginning February 1, 2003.  Please read the instructions prior to completing the EFS form (Excel spreadsheet which includes appendices as worksheets).  Changes since the last revision are made in red.
