| A REMINDER TO ALL OWNERS AND CONSULTANTS OPERATING OR PLANNING A
REMEDIATION INVOLVING UNDERGROUND INJECTION OF LIQUIDS: If you are injecting ANY
liquids or liquid mixtures (ORC, microbes, nutrients, treated water, etc.) into the ground, you must
provide OPS with a copy of your Underground Injection Control (UIC)
permit. These permits are obtained from the EPA Region 8 office in Denver
(303) 312-6312.

Note: If you are extracting groundwater and a permit is
required to discharge or dispose. You must provide OPS with a copy of the
permit.
You may submit the copy (ies) of the permit(s) with your Corrective
Action Plan (CAP) or with our CAP Implementation Report.
